- Open ‘My Chart Designs’.
- Choose your design and click on it.
- Click on ‘Birth submission form’.
- Click on ‘Settings’.
- Click on ‘Display agreement checkbox’.
- Click on ‘Make the agreement checkbox optional’.
- Save.
